Support Manager - Young Persons Service
The Young Persons Support Manager will be key in continuing our goal of specialising our “Young Persons” (13 – 18) offering at a time we are emphasising our dedicated Learning Disabilities service alongside our Award-Winning Care Farm. With considerable scope for growth, you will not be afraid to ‘get stuck in’, whether it be to create tangible positive changes in our clients’ lives, making key contacts in the community or in mentoring our aspiring Support Workers on their own care journeys. Your Role
Key tasks of a Young Persons Support Manager include:
Manage your own caseload of clients for whom you will conduct thorough needs assessments, prepare care, and support plans which reflect the client’s needs
Participate in the management, recruitment and personal development of Support Workers
Acting as an advocate for your clients by working with their support teams and family to ensure the best quality of care for everyone is achieved.
About You
Skills and experience for a Young Persons Support Manager includes:
Experience working with and improving the lives of people in the community
A relevant professional qualification such as social work, occupational therapy, psychology, speech and language therapy or nursing is highly desirable.
Proven experience of undertaking assessments and reviews
Prior care management experience is essential.
