What is the London Care Record?
The London Care Record is an up-to-date and secure digital record of a person's health and care information over time and across different parts of the NHS and social care services.
You can also access the Universal Care Plan from the London Care Record.
These tools give you the information you need about a resident such as their medications, previous medical history, hospital discharge summaries, London Ambulance Service case summaries, clinical correspondence and plans for their care.
The London Care Record can be accessed through a number of digital care planning systems.
This instant information will support you to provide safer, faster and more joined-up care for residents while also saving you valuable time.
Over 100 care homes in the Capital already use the London Care Record and their staff report that this helps:
- provide more effective care to residents;
- reduce unnecessary ambulance call outs and hospital admissions;
- support faster discharges from hospital;
- and so much more.
How to connect to the London Care Record?
To be eligible to connect to the London Care Record your care home needs to:
* meet the Data Security and Protection Toolkit standards;
* use Person Centred Software, Nourish, Care Vision, Log My Care or Carebeans care planning systems.
