We are looking for a proactive and forward-thinking Care Coordinator who wants to move beyond traditional admin-based roles. You will have the opportunity to improve systems, support better outcomes, and play a key part in delivering high-quality, person-centred care. Using modern systems that reduce administration, you will focus on building relationships, making informed decisions, and ensuring everyone receives the right support.

Location: Crouch Hill, North London (office-based with flexibility) Hours: Full-time (37.5 hours per week + emergency phone rota) Key Responsibilities • Develop and review person-centred care plans • Coordinate care packages, staffing, and rotas • Match support workers based on skills and compatibility • Act as the main point of contact for families and professionals • Respond to changes and resolve staffing challenges • Ensure services meet CQC and safeguarding standards • Maintain accurate records using digital systems • Contribute to service improvement and team development

Essential: • Experience in care coordination or supporting individuals with complex needs • Strong understanding of person-centred care • Excellent communication and organisational skills • Ability to remain calm and make decisions under pressure • Comfortable using digital systems • Knowledge of safeguarding • Right to work in the UK and willingness to undergo an enhanced DBS check Desirable: • Level 3 qualification (or above) in Health and Social Care • Experience in CQC-regulated services • Experience coordinating staff or rotas
