Prevention and Wellbeing Coordinator

An exciting opportunity to be part of our Prevention and Wellbeing Team where you will support individuals to achieve objectives to improve their health and wellbeing, alongside supporting local communities to coordinate and grow what is on offer in the local area. Working in a creative, strengths based and personalised way you will support local residents who have had contact with Adult Social Care and their carers to find practical and sustainable solutions to achieve their objectives

Your Role

You will be experienced in approaching issues in stages and developing plans. Coordinate information about what already exists within our local communities, identify gaps and opportunities, develop new initiatives and encourage the local community to take lead on managing this long term. Build strong relationships with local partners, understand the value of residents and provide support to increase accessible community initiatives. Manage a diverse case load while coordinating and expanding the community offer. Service users have told us that they feel listened to, more in control, and have managed to achieve numerous personal objectives. Many have gone on to be active members of local communities, leading and attending new initiatives or becoming part of those already established.

About You

Passion for working with people, prompting self-management & independence .Clear understanding of boundaries, risk assessment and personal accountability. Understanding of Health &Social Care within Barnet. Experience of working with adults with care and support needs. See the fit with developing local landscape &National picture. Knowledge of relevant social care legislation/ guidance. Understanding of protecting and making vulnerable adults safe. Excellent interpersonal skills. Ability to use wide range of IT applications and document systems. Experience of recording& reporting on data. Creative & enthusiastic approach to support individuals build good networks
